- Print required forms for Bridgeway Services Participation appropriate to your situation. Please bring your completed forms to your scheduled initial consultation.
Important Information: Because our Master's level staff runs the afternoon social skills groups, afternoon time slots are limited for initial consultations. You will get a quicker time slot if you are available to meet during the school day or summer. We do not allow students to try out a group as part of the initial consultation process until we have had a chance to meet the student and determine the best group placement.
